Player Registration Page

In order to participate (play & volunteer) with the Jacksonville Axemen, or JaxAxe Women’s, please fill out the following:

1. PLAYERS ONLY – Register with the USARL (the National Governing Body) and pay their registration fees REGISTRATION OPENS May 1
2. Sign and initial the participation agreement and liability waiver. Please print and return to the team.
   • Initial top left corner of each page
   • Circle Men’s or Women’s (top right of 1st page)
   • Enter Current Season (highlighted)
   • Sections 1-6: Initial each section indicated (highlighted)
   • Section 7: Acknowledge that you DO or DO NOT have personal medical insurance by checking YES or NO
   • Print name, sign, and date
     • View participation agreement and liability waiver.
